Enrollment

To enroll your child/children in our program follow the steps below:

  • Complete an enrollment application by clicking on the link below.
  • Provide a copy of your child’s birth certificate
  • Current Physical & Immunization form no more than 30 days old
  • Provide Allergy Plan or Asthma Action Plan if applicable
  • Contact the Director and/or Office Representative to schedule an Orientation and discuss fees and other important information regarding the program

Completing the enrollment application for the program does not guarantee space. Once all licensing documents have been provided and approved, you will be notified of enrollment and payment schedule.
